Fraud questionnaire

It's important to let us know about any potential instances of fraud, so thank you for taking the time to fill out our questionnaire. It should only take a minute or two to complete, but please do provide as much information as possible.

Once you have submitted the questionnaire, our fraud team will investigate and will get back in touch with you if necessary. 

Your application number is noted near the top of some of the correspondence you've received from us about your agreement, such as your welcome email or an annual statement. Additionally, if you have a Direct Debit set up, your agreement number is shown in the reference on your bank statement.
